Survey on Member Role and Interest in Innovative Models Closes Feb. 20

Join your peers in telling us what innovative models you’ve been involved in and what your experience has been. Not currently testing new payment or service delivery models? We want to know if you are interested in learning more and/or participating in these or other models in the future.

We also want to hear from you about what resources you need to be successful under these models. This survey is applicable to all provider types whether you are involved in a federal bundled payment program or Accountable Care Organization, a state initiative to better coordinate care, or are partnering to offer services on your HUD housing campus. Please take a 15-20 minutes today to answer a few questions as part of our member survey and help us build our knowledge. 

Your input will help us inventory current activity and educate policymakers and members on the types of models and initiatives our members are engaging in. It will also help us advocate for LeadingAge's integrated services vision for America's Aging Population and opportunities for our members to lead new payment and integrated service models.

LeadingAge will share the results with members at the PEAK conference in March and through other communications. We will also be developing case studies on some of the models we learn about through the survey.
