CAST Case Study: Evangelical Homes of Michigan

In July 2013, CAST Patron and LeadingAge Member Evangelical Homes of Michigan (EHM) learned about a newly formed, physician-based accountable care organization (ACO) called Northwest Ohio ACO during its conversations with Ohio Presbyterian Retirement Services (OPRS), a LeadingAge member in Columbus, OH.

Northwest Ohio ACO wanted to serve clients in both Ohio and Michigan. However, the only way to provide this 2-state access to services was to establish a “conduit relationship” with an organization that was licensed to provide home health care services in Michigan.

To meet this need, EHM established a partnership OPRS. That partnership, the services it provides to ACO clients, and the enabling technologies utilized is the subject of a new CAST Case Study entitled “Collaborative Partnership Expands Service Offering and Reduces Hospitalizations.”

Components of the Partnership

OPRS participated in the partnership through its home care and hospice subsidiary, Senior Independence. EHM participated in the partnership through 3 of its wholly owned subsidiaries:

  • Shared Services Home Health Care, a certified home health care agency.
  • Shared Care Services, a private-duty home care and geriatric case management services.
  • LifeChoice Solutions®, a technology, solutions and home adaptation company.

The partnership allowed the 2 providers to offer a full array of services to ACO clients in both Michigan and Ohio. This helped the ACO create a 1-stop, 1-phone-call relationship with clients in both states. 

To date, client satisfaction exceeds the joint partners’ expectations and hospital readmissions in the 30 days after hospital discharge are ranging between 3% and 6%, which is very impressive compared to a national average readmission rate of 19% for Medicare patients.
