CAST Case Study: PointClickCare’s 7-Stage EHR Implementation Model

A new CAST case study explores how PointClickCare, a LeadingAge Silver Partner with CAST Focus, helped Avante´ successfully adopt and utilize the advanced functionalities in its electronic health records (EHR) system. 

Avante´, which provides skilled nursing care at 20 sites in Florida, North Carolina, and Virginia, wanted to drive quality across all of its care settings and maximize the efficiency of its administrative teams.

7-Stage Adoption Model

The project’s implementation approach followed PointClickCare’s 7-Stage Adoption Model. Each stage of the model is timed to ensure that a solid foundation is in place before the team moves onto the next stage. 

The 7 implementation steps include:

  • Fundamentals: Admission, Discharge, and Transfer (ADT); Minimum Data Set (MDS); Care Plans; Billing; and Clinical and Administrative Dashboards
  • Basic EHR: Assessment Reference Date (ARD) Planner, Therapy Integration, Ancillary Imports, Claims and Collections
  • Point Of Care (POC) Documentation: POC, Tasks and Flow Sheets, and Alerts
  • Advanced Clinical Documentation: User Defined Assessments (UDAs) and Structured Progress Notes
  • Orders Management: Order Entry, Integrated Medication Management (IMM), Pharmacy Integration, and Electronic Medical Administration Records (eMAR)
  • Physician Engagement: Physician Portal
  • Interoperability: Health Information Exchange and Personal Health Record data exports.

Outcomes

Avante´ quickly experienced positive outcomes as a result of following the 7-stage model, according to the case study. The organization was able to:

  • Increase EHR utilization across settings by 80%. 
  • Eliminate duplication of reports by transitioning to a paperless process.
  • Improve productivity and efficiency through use of a cloud-based system.
  • Triple the number of settings capturing data at the point of care. This improved work flows and increased the level of care being provided to residents. 
  • Reduce medication errors and avoid adverse drug events by integrating pharmacies into the PointClickCare system.
  • Improve occupancy rates by increasing the use of Intake and Referral Management (IRM) by 120%.
