Providers who received Provider Relief Funds (PRF) or American Rescue Plan (ARP) Rural payments between January 1 and June 30, 2022, were required to report on their use of these funds by September 30, 2023. For those who failed to report due to extenuating circumstances, HRSA is offering a one-time opportunity for these providers to request the ability to report late.

To submit requests for late reporting, fill out the DocuSign form by October 27.  If the request is approved, providers will submit their RP5 reports between November 13 and December 1, 2023.
