Now published on iQIES, the Preview Calendar Year 2025 (CY25) Annual Performance Reports (CY25 APRs) for the expanded Home Health Value-Based Payment (HHVBP) Model are available for home health agencies (HHAs); Centers for Medicare and Medicaid Services’ (CMS) instructions to access them are here.

The CY25 APRs provide the HHA’s Annual Payment Percentage (APP) based on CY24 performance year. These payment percentage adjustments will be applied to Medicare Fee-for-Service (FFS) claims with through dates in the CY26 payment year.

In addition, the CY25 APRs provide information related to HHA performance on key metrics that feed into the HHA’s APP, including performance measure scores, Improvement Points, Achievement Points, Care Points, and the Total Performance Score (TPS).

Importantly, HHAs who find evidence of of an error in their report have 15 calendar days, or until September 5, 2025, to submit a recalculation request.
