LeadingAge and its Post-Acute Care (PAC) Coalition partners on November 12, 2025, submitted “Medicare Advantage Prior Authorization in Post Acute Care: Issues and Interventions” and an accompanying proposed standard Medicare Advantage (MA) Prior Authorization (PA) request form to the Centers for Medicare and Medicaid Services (CMS). The 12-page PDF includes two detailed solutions that illustrate approaches to standardize Medicare Advantage (MA) prior authorization requests across all plans for PAC services and to speed up the decision-making on these requests.  Both the report and the form are below.
