The Department of Health and Human Services Office of Inspector General (OIG) released their report to Congress on July 13, 2026, covering activities by the office between October 1, 2025 and March 31, 2026. During this time, the office conducted 881 investigations, more than 1,100 criminal referrals, and more than 1,200 individuals and entities were excluded from participating in federal healthcare programs.
Through their focus on Medicaid fraud, the OIG oversees the work of Medicaid Fraud Control Units (MFCUs) within each states’ attorneys general office. OIG estimates they’ve recovered approximately $2 billion in conjunction with MFCUs, combined for nearly 1,200 convictions, and more than 900 entities added to the exclusion list. The report also contained brief reviews of work on improving fiscal integrity, addressing the opioid and prescribing crisis, adding safeguards to federal grants and contracting, and improving cyber security infrastructure. The office suggested review of nursing home antipsychotic use under a section titled ‘Protecting People from Harm,’ and mentioned the nursing home Special Focus Facility program as a failure.
