Fourth Annual Conference

Back for a fourth year! Continuing Care at Home (CCaH) programs continue to drive meaningful change and provide an option for what most active older adults want, to age in place. As Life Plan Communities (LPC) and Continuing Care Retirement Communities (CCRC) look to diversify their portfolios, CCaH program is a growing, profitable model and offers a new entry point into communities.

  • New Exploratory Track

    New in 2024 is a track for CCaH Exploration, which will include four 1-hour sessions specially targeted for individuals and/or organizations who are interested in having more in-depth information on how CCaH programs work and what’s involved in starting one.

Hospital at Home: Acute Care Reimagined

During the COVID-19 pandemic, the Centers for Medicare & Medicaid Services and state health officials eased some regulatory restrictions to help healthcare organizations build their capacity, increase access to care, and reduce burdens on a strained workforce. These regulatory changes paved the way for a new program that provides acute-care services to patients in their homes. This session will present the story of Nascentia Health, a home care provider based in Syracuse, NY, which worked with a hospital partner to develop an Acute Hospital Care at Home (AHCaH) program. Presenters will explain how the AHCaH program works and the steps necessary to create and execute the program. Learn how to weigh the benefits and risks of an AHCaH program, choose the right partners, and measure outcomes.

Actuaries, Risk Assessment, & Removing Barriers to Understanding CCaH Model

Continuing Care Actuaries has assisted in the development of two of the first four CCaH programs and currently works with approximately 80% of the existing operational programs. Learn how the CCaH model works to better understand how it can provide an alternative to the traditional CCRC/Life plan model.

Serving the LGBTQ+ Population

Prepare your CCaH program with the tools needed to reach the untapped LGBTQ+ demographic. The premiere organization for LGBTQ+ seniors, SAGE, will provide the necessary educational training for CCaH staff, prospects, members, and care givers to assure inclusivity and belonging for your organization.

Medicare Advantage

Understand the lesser known benefits that Medicare Advantage plans offer to their policy holders and how it works with the benefits of CCaH programs.

LPCs & CCaHs Working Together

Though the continuing care at home model began over 30 years ago, some of the challenges in expanding these programs have often stemmed from concern that it will dissuade seniors from moving into a life plan community. Three organizations, from an original program, an eight-year-old program and a program established in 2023, will share their experiences describing their expansion of the organization’s mission, market territory and revenue diversification. The panel will share various case studies supporting the symbiotic relationship of the program to the community as well as a discussion of the benefits and challenges of working together with the life plan communities, energizing the wait list, developing bridge programs, and integrating wellness programs with the communities.

Enhancing Member Benefits through Concierge Services

This session explores how personalized concierge services can elevate the value of aging-in-place programs by offering members a tailored experience to meet their non-medical needs. Highlighting specific services and their impact, the session will delve into members’ enriched experiences and Thrive at Home and LiveWell by Blakeford’s decisions to build partnerships that deliver exceptional satisfaction.

Enhancing Independence: Technology for Everyday Living

Unlock the potential of technology to elevate the quality of life for older adults in their own homes. This session delves into innovative solutions that bolster independence and ensure safety, fostering enriched social engagement and enhanced well-being. Participants will gain insights into practical tools that not only empower older adults to confidently manage their personal care but also to nurture vital social connections. Discover how embracing these technologies can lead to a life of enriched autonomy and meaningful interactions with the community.

State Oversight & Governance

Each state oversees and regulates CCaH programs differently. There are some states that do not have regulatory guidance established. Learn about the differences in state oversight and how you can work with state LeadingAge associations and local leaders to advocate for proper regulatory oversight so that you can successfully launch and operate a program.

Importance of Compassion in Journey of Aging

Aging is a journey. A person’s needs can change rapidly due to a major health event such as stroke or their change in needs can be much more gradual. Everyone’s journey is different and unique and there is no crystal ball to know how the journey will go. In this session, we discuss information from the book “Compassionomics” by Dr. Stepehen Trzeciak and Dr, Anthony Mazzarelli on the scientific evidence that caring makes a difference. We will then discuss what role CCaH programs can play in adding more compassion to the medical, psychological, social, and spiritual needs of older adults.

To age in your home independently, planning is essential. Your plan should include how and where you want to grow older. It should advocate for you, respect your choices and help you stay healthy and independent while remaining in your home. In this session, hear directly from 4 members of Continuing Care at Home programs across the country.
