We provide individualized care and services for you or your loved one to support your specific physical and medical needs.  Depending on your care plan, you may choose one of three residential settings on our campus – our Northern Virginia Health Care Center or one of our two assisted living facilities – Willow Oaks Assisted Living and the District Home Assisted Living.

Our Health Care Center is a "5 Star" facility as rated by the Federal Centers for Medicare and Medicaid Services, which is one of the highest ratings of any facility in Northern Virginia.
