Eliza Bryant Village, established in 1896, is the oldest operating African American long-term care facility in the United States.  In 2012, we were designated as a Critical Access Facility, one of only seven skilled nursing facilities in Ohio.

Eliza Bryant Village is located on 17 acres in Cleveland, Ohio.  We serve over 1,200 seniors each year that depend on The Village to enhance the quality of their lives.

Our unique programs and services provide seniors with opportunities to remain independent, receive the care they need, and provide social, medical, therapeutic and recreational activities that help seniors remain living in the community.  Our core programs include:

• Inez Myers Senior Outreach Center – Adult Day, Transportation, Nutrition and Well Senior Programs.
• Low-Income Senior Housing – 149 independent HUD 202 units ranging from apartment style to cluster homes.
• The Rehab Center – Specializes in providing short-term care and helping individuals return home after an illness or injury.
• 175 Bed Skilled Nursing Facility – Skilled nursing, memory and total care.

 

The  Senior Outreach and Adult Day Program is the largest in Cleveland. In 2013, we transformed from a social to a medical model to better meet the needs of seniors. The goal is to delay or prevent seniors being institutionalized by offering programs to help them remain independent.

The Rehab Center provides comprehensive care for individuals who require skilled care and rehabilitation services including physical, occupational and speech therapy; and specialized medical and nursing services.  The goal is to return individuals to their previous level of independence.
