Updated Shared Care Planning and Coordination Portfolio

CAST has updated its Shared Care Planning and Coordination Online Tool with updated vendor information for 19 products and a new case study. Improving Care Coordination and Data Sharing with Acute Care and Payers illustrates how integration technology can help a provider reduce gaps in care and remain a preferred provider to a health system. CAST Supporter Netsmart and Landis Communities partnered on this successful project.
 
LeadingAge CAST produces and publishes six Technology Selection Tools that help providers understand, plan for, select, implement, and adopt the appropriate technology while advocating for innovative care models. In addition to the Shared Care Planning and Coordination Technology tool, the topics include EHR, Telehealth and RPM, Medication Management, Functional Assessment and Activity Monitoring, and Social Connectedness and Engagement Technology

CAST Commissioners Prepare for Healthcare Industry Transformation

CAST Commissioners gathered for their biannual meeting on March 18, 2018, in conjunction with the PEAK Leadership Summit, and discussed how best to prepare for the coming healthcare transformation. Together, they explored ways that senior living providers can shift mindsets, processes, and business models to become providers of choice.
 
CAST Commissioners and guests also evaluated concepts for proposed demonstrations that would encourage future technology-enabled care models in long-term and post-acute care (LTPAC). The group carefully considered these concepts, sharing their organizations’ perspectives and charting next steps for CAST and its members. 

CAST Case Study: Skilled Nursing Residents Adopt the Birdsong Tablet

A new CAST case study explains how LeadingAge CAST Patron Westminster-Canterbury on Chesapeake Bay created a new custom touchscreen digital tablet for social engagement at an affordable cost, in partnership with TMM Software. Thanks to local youth volunteers who helped train residents, the tablet is achieving a high acceptance rate.

CMS Proposes New Rules

The Centers for Medicare & Medicaid Services has proposed rules that would raise payments to skilled nursing facilities and hospices, simplify patient classification, encourage interoperability, and increase patient access to data. CMS is accepting comments until June 26, 2018.
