Continuing Care at Home (CCaH)
Continuing Care at Home (CCaH) programs continue to drive meaningful change and provide an option for what most active older adults want, to age in place. As Life Plan Communities (LPC) and Continuing Care Retirement Communities (CCRC) look to diversify their portfolios, CCaH program is a growing, profitable model and offers a new entry point into communities.

Exploratory Track
A track for CCaH Exploration includes four 1-hour sessions specially targeted for individuals and/or organizations who are interested in having more in-depth information on how CCaH programs work and what’s involved in starting one. There will also be a pre-conference session/forum for those providers who want to ask specific questions about starting CCaH programs.

Marketing Operations
This track offers data-informed strategies to strengthen marketing and operational effectiveness. Topics include omni-channel outreach, rural market engagement, and shifting payer and regulatory dynamics in HCBS.
Wellness Coordination
This track offers practical approaches to promoting health, independence, and quality of life. Sessions will explore the integration of supportive technologies, Medicare Advantage navigation, and innovative partnerships that enhance member well-being.
CCaH Exploration
This track offers foundational insights for those exploring or newly engaged with Continuing Care at Home (CCaH). Content will address key operational considerations, collaborative approaches, and trends in long-term care utilization.
Your HCBS Partners are Sitting on Shifting Payer and Regulatory Sands
Continuing Care at Home programs rely on their home and community-based partners to support individuals to stay in their homes long term. Partners from adult day to home care, to PACE have a role to play in keeping CCAH members independent. But what happens when those partners experience outside regulatory and payment pressures? Shifting payments and regulations can decrease HCBS providers’ ability to offer consistent services. Understanding the pressures your HCBS partners face at the federal and state level will help you be nimble in supporting and meeting members’ needs while also helping your partners create sustainability and stability.
Finding That Needle in a Haystack: The Unique CCAH Buyer
CCAH is still new in the senior living world as an option for retirement living. Who is that person that chooses to stay at home but still wants the support that communal living offers. And once you find them, how do you speak to them in a way that resonates with them? We will look at two organizations, Kendal at Home, one of the most well-established CCAH models, and Wesley SecureCare, focusing on how to find those most likely to become members and then use omnichannel approaches to reach them.
Reaching Rural Consumers: The LSA/LTSS RAAN Project
In 2022, Lutheran Services in America (LSA) created the Rural Aging Action Network (RAAN) as a national collaborative dedicated to expanding community-based services and supports to ensure independence and choice for older adults living in rural communities. LSA selected five provider organizations from four states to join the LSA RAAN Collaborative and help them build the infrastructure to address gaps in care for older adults. The LeadingAge LTSS Center UMass Boston partnered with LSA to evaluate the RAAN’s effectiveness, impact, and value. This session will provide an overview of this innovative community-centered model and highlight findings from the evaluation.
CCaH Benchmarking Survey Updates
Get the latest updates on industry benchmarking data from member demographics, sales conversion rates, to care utilization. This data is valuable to better understand where we are as an industry and can be used to take back to your organization’s higher leadership and board for education on the model. Our data can tell a story about our programs, which is why it is important to track certain information. We will also compare 2024 data to prior years.
Newer Cognitive Testing Technology
“There are four types of people in the world. Those who have been caregivers. Those who are caregivers. Those who will be caregivers. And those who will need caregivers.” – Rosalynn Carter. This is a core premise behind the promise of memory support at Willow Valley Communities. At the Marlin and Doris Thomas Memory Center, opening in 2026, a graduated array of amenities and services will line the pathway for people living with dementia and their care partners. The Adult Day program will be an essential element for promoting socialization and preventing burnout, while the Brain Tech Center, Resource Lounge, Education Room, and Assessment Center will be dedicated spaces for the support, training, and education available and accessible to informal care partners from the community. Learn about the ongoing engagement and activation efforts designed to improve the confidence and comfort of care partners and how this new design will reinforce the model of Continuing Care at Home in our communities.
Bringing Concierge Services to CCaH Members with Dorvie

With trusted, vetted providers, transparent pricing, and consolidated billing, Dorvie provides a seamless, reliable experience for both members and staff – helping older adults live well and thrive independently at home.
The Montreal Cognitive Assessment (MoCA)
In this session, Dr. Murray Gillies will discuss the Montreal Cognitive Assessment (MoCA) test and compare it to various other scales used in everyday practice. He will present the various digital tools from MoCA and discuss how they compare to the paper tests. In particular, the XpressO tool from MoCA will be highlighted and results from both online use, as well as in clinics will be shown. Aspects such as test-retest reliability and effects of language and device type will be explored. Dr. Gillies will also share some experiences of integrating tools like the XpressO into the clinical workflow of the MoCA clinic.
Medicare Advantage – From Policy to Practice to Pitfalls
Ever have a prospective CCAH member say they “don’t need CCAH because they have Medicare Advantage (MA)?” Join LeadingAge MA expert Nicole Fallon for a guided exploration of how Continuing Care at Home programs can help their members better understand and maximize their MA benefits. The session will include a national overview of the trends, news and controversies that the MA field has experienced in recent years and the implications for both providers and Medicare beneficiaries. CCAH program leaders will also learn how to help their members navigate the program limits and obligations of MA plans, especially the supplemental benefits offered by these plans.
SilverSmart Technology for Everyday Living
Technology has become part of our everyday lives, it impacts how we work, communicate, learn, travel and play. Technology in healthcare has changed lives in more ways than could have ever been imagined. But adoption and acceptance of technology with older adults and in aging services is lagging. For over 15 years, Lutheran SeniorLife has been bridging the gap between technologies designed to assist elders with living in place and Living an Abundant Life and those we serve. The SilverSmart program brings awareness, education, hands-on interaction and support with assistive technologies that support living at home safely while remaining active and engaged. In addition to learning about the design, purpose and goals of the SilverSmart program, you will hear examples of the assistive technology devices available in the center, including their successful adoption and positive outcomes.
Creating Partnerships to Expand Missions and Member Benefits
Wesley Inc established Wesley SecureCare a unique standalone organization offering Baltimore’s only CCaH program. Edenwald Senior Living is a forward-thinking single site Life Plan Community in Towson, Maryland. When the two leaders met it seemed like an obvious win-win for each organization to enter a partnership. The Wesley would be able to offer members access to enhance membership benefits through community amenities and have administrative support for efficiencies, and Edenwald who wanted to start a CCaH program of their own to reach more members of the community and expand their mission would be able to do so without a large investment and resources. In this session, hear each organization’s story of why, what is working, and what is still a work in progress as the organizations try to move toward an affiliation agreement.
CCaH Programs Partnering with Villages
Similar in many ways to CCaH programs, Villages are community-based, nonprofit, grassroots organizations formed through a cadre of caring neighbors who want to change the paradigm of aging. Local Villages connect members to a wide array of practical support services and social connections that enable older adults to enjoy a rich, independent and healthy quality of life when they choose to age in their homes and communities. Villages build a sense of community and offer resources, services, programs and activities, including social and educational programs, health and wellness activities and volunteer assistance with transportation, light home maintenance, and technology coaching. At this session you will hear from the Village to Village Network which represents over 300 Villages nationwide, along with a collaborative partnership between a CCaH program and its local Village.
How to Build a CCaH Operation From the Ground Up
While many aging services sponsor organizations are exploring the CCaH model to reach and serve more older people, as well as a strategy to growth venue and diversify services, many interested organizations are unclear about what starting a CCaH program looks like and what this process entails. At this session you will hear from two colleagues – one who has consulted with numerous start-up CCaH programs and another who recently launched a new, very successful CCaH program – about the typical steps that it takes to start a new CCaH program from the ground up and what to anticipate in your CCaH journey.
Ask an Actuary: Trends in Long Term Care Utilization
Baby Boomers, now aged 61 to 79, represent a growing demographic that is reshaping the senior living landscape. Brad Paulis and Tyler Zentz of Continuing Care Actuaries will explore how this generational shift is impacting the senior living industry and how organizations must adapt to meet the evolving expectations of this key population. Topics will include trends in demand for senior services, the advantages of CCAH programs, the financial benefits and challenges associated with these models, opportunities for affiliations in developing at home programs, and strategies to reduce the cost of delivering care through a CCAH program.

This humanistic AI is one of the most effective and widely used approaches for creating positive change, building trust, and improving the outcomes of your conversations.

It should advocate for them, respect their choices and help them stay as healthy and independent as possible while remaining in their long-standing homes. In this closing conference session, hear directly from a diverse group of Continuing Care at Home program members from different corners of the country. Discover why they chose the CCaH program model as their preferred plan and strategy to help them age gracefully and safely with dignity in their own homes.
