Centers for Medicare & Medicaid Services (CMS) issued clarification on measure exclusions for patient transferred or discharged to hospice as part of the home health value-based purchasing (VBP) model. The following measures used in the home health VBP will exclude patients discharged to hospice as of January 1, 2023:

• Improvement in Dyspnea
• Improvement in Management of Oral Medications
• Total normalized composite for change in mobility
• Total normalized composite change in self-care
• Discharge to community – this measure also includes individuals discharged to institutional hospices as well as home based hospice
