In the MLN Connects newsletter issued September 4, the Centers for Medicare & Medicaid Services (CMS) clarified Medicare signature requirements for providers signing off on care.
In the updated PDF, “Complying with Medicare Signature Requirements,” (which is dated July 2025) CMS clarified that rubber stamp signatures are only permissible in cases where the signer has a physical disability that interferes with the ability to provide a handwritten signature.
Further, CMS clarified that a physician or non-physician practitioner (NPP) signature is required to authenticate medical records documents that have been transcribed by another party, including artificial intelligence or dictation software. Remember that Medicare claims may be denied in cases where the medical records lack appropriate signatures. Read more here.
