About the Workgroup

Through regular meetings and resource dissemination, this highly engaged workgroup led by LeadingAge national office staff—with input from subject-matter experts and LeadingAge member organizations—provides participants with opportunities to explore the relationship between quality metrics and payment in the expanded Home Health Value-Based Purchasing (HHVBP) Model. Participants in this free-to-all LeadingAge members initiative will:

  • Identify the key metrics that influence quality measures.
  • Understand the various factors influencing yearly payment adjustments, including providers’ improvement in quality measures, and peers’ performance.
  • Explore opportunities to enhance quality improvement processes through evidence-based practices.
  • Identify actionable improvement strategies from peers’ performance successes.
  • Gain insight, through data sharing and analysis, into the relationship between populations served by LeadingAge’s nonprofit and mission-driven members and HHVBP model outcomes.

Note: Robust participation in data sharing and engagement is critical to the achievement of the workgroup’s goal: helping members improve their quality measures and reimbursement levels.

Workgroup Information

The goal of this workgroup is to gain, build, and exchange ideas through a process of evidence-based learning and open dialogue.

  • The workgroup will meet via Zoom the final Tuesday of every other month at 3pm ET. Register here.
  • Data driven analysis is critical to the success of the workgroup, therefore participating agencies will be requested to submit quarterly data from their Interim Performance Reports (IPR) as well as data from their Annual Performance Reports (APR) deidentified and aggregated to show the impact of workgroup’s efforts.
  • To enable participants to share and learn from each others; experiences, meetings will be highly interactive and include presentations as well as small- and full-group conversations. The introduction presentation of each meeting will be recorded; however, open discussions will be not recorded and participants must be present to earn the benefits of their peers’ experiences.
  • Any team member is welcome to participate; however, this workgroup would highly benefit quality managers, clinical managers/leaders, program managers, as well as billing or finance staff.
  • To foster engagement and community-building, participants will be encouraged to be on camera and be present for the full duration of all meetings.
  • To encourage continued improvement efforts outside of workgroup interactions, suggested readings, videos, and other resources will be shared between meetings on the identified areas for improvement.

LeadingAge would like to better support members in the HHVBP model. We are therefore collecting data from members on their current performance in the program. This information will remain deidentified and aggregated to show the impact of the model on nonprofit and mission-driven providers. LeadingAge will utilize this information to develop education and advocacy to improve members standing and preserve access for beneficiaries.

Of the 10 quality measures home health agencies are accountable to in the HHVBP model, the following were identified from the CY2023 data released by CMS as the most actionable for LeadingAge member improvement:

  1. OASIS-Based Measure: Discharge Function Score
  2. Claims-Based Measure: Home Health within-stay Potentially Preventable Hospitalization (PPH)
  3. CAHPS-Based Measures: Special Care Issues

Getting Started with HHVBP Resources:

It is critically important for members to understand the recalculation request process as it is the only way agencies can appeal their standing in the HHVBP program.

Home health agencies may submit a recalculation request if they believe there is an error in their quarterly IPR or APR. Recalculation requests do not apply to errors with data submission.

As Director, Home Care and Hospice Operations and Policy, Katy is responsible for LeadingAge’s regulatory and operational efforts in the area of home care, home health, palliative care and hospice. She meets with federal and regulatory officials and represents LeadingAge on various strategic committees both internal and external. Katy is primarily responsible for developing comment letters on behalf of LeadingAge and its members for rule making, providing operational support for members.
