The Department of Housing and Urban Development (HUD) on April 17, 2026, published updated forms for use related to the Violence Against Women Act (VAWA). HUD has confirmed to LeadingAge that the new and updated HUD-approved VAWA forms are for owners and agents to begin using immediately.
Compliance will be reviewed during Management and Occupancy Reviews (MORs), with findings possible for use of outdated forms. No additional HUD approval is needed for housing providers to update their written procedures, as applicable, to align with the new forms.
The forms are available here (VAWA forms are numbered HUD-5380 through HUD-5384). LeadingAge has been monitoring the development of these forms since 2025, when an earlier version was published by HUD.
In response to questions from LeadingAge members about the “housing advocate” field in the forms, HUD stated that the intent of the ‘housing advocate’ information section is to provide residents with reliable contact information for confidential support. If a consistent local contact is not available, it is acceptable to include general, stable resources such as the National Domestic Violence Hotline or other state/local advocacy organizations without naming a specific individual. National resources alone are sufficient if the information is current and accessible.
In its stakeholder message, HUD recommends that housing providers direct questions to local field offices, as well as the HUD Office on Gender-Based Violence (ogbv@hud.gov).
