GUIDE experts from CMMI will explain why providers should consider participating in this new voluntary model to support people living with dementia and their families.
An August 30 live event from the LeadingAge Learning Hub is essential for LeadingAge members interested in the Guiding an Improved Dementia Experience (GUIDE) Model.
The live event, free to LeadingAge members, will provide an overview of the GUIDE Model, recently announced by The Center for Medicare and Medicaid Innovation (CMMI). The model includes a comprehensive package of supports for people with dementia and their caregivers: care coordination and management, caregiver support and education, and respite services. Its purpose is to delay avoidable long-term care placement and enable more people to remain at home if that is their preference.
Three members of the CMMI GUIDE Model team will present at the live event:
- Sarena Ho, health insurance specialist
- Emily Johnson, GUIDE model lead
- Tonya Saffer, division director, Division of Healthcare Payment Models
The CMMI team will be joined by two LeadingAge staff: Nicole Fallon, vice president, integrated services and managed care, and Mollie Gurian, vice president, home-based and HCBS policy.
GUIDE is an eight-year voluntary model focused on dementia care in the home and community. Participants must be Medicare Part B providers with expertise and capabilities to provide ongoing longitudinal care and support for people living with dementia and those who care for them.
