LeadingAge CAST convenes this Summit that brings LTPAC providers together with acute care providers, payers, health IT vendors, health information exchange intermediaries, consultants, system integrators, and solution providers to address how technology is connecting the healthcare ecosystem in the era of health and payment reforms. Summit education content will address innovative care delivery and payment models enabled by technology in general, and the broader health IT, interoperability, and health information exchange modalities in particular. This year, the summit will feature several real-world cross-sector telehealth, health information exchange and care coordination, as well as exploration of the role of venture capital in shaping the Collaborative Care Eco-System, especially in light of challenges and opportunities created by COVID-19.
