Telehealth use has seen a significant increase among care providers, including LeadingAge members and other long-term and post-acute care (LTPAC) providers. However, little information is shared with other providers who are interested in exploring telehealth and the best approaches, uses, cases, and solutions to consider.

This webinar will feature two provider use cases, in the first of which Synchronous Telemedicine/ Telehealth is deployed in a Skilled Nursing Community to improve access to physicians, reduce hospitalizations and re-admissions, and improve outcomes. Whereas in the second, Remote Patient Monitoring (RPM) is being in Home Care to drive staff efficiencies, support care management, including chronic care management, and enhance engagement, even socialization!
