The Provider Relief Fund (PRF) Reporting Portal is open for providers to submit their Reporting Period 5 (RP5) reports. These reports must be submitted no later than September 30, 2023, at 11:59 p.m. ET but providers are encouraged to get them in earlier.

RP5 is for providers who received one or more PRF (general or targeted) and/or American Rescue Plan Rural payments exceeding $10,000, in the aggregate, from January 1, 2022, to June 30, 2022. These reports are required to comply with PRF Reporting Requirements.
