From CMS: New Tracking Module for Managing Provider Complaints
The Centers for Medicare and Medicaid Services (CMS) on December 22, 2025, implemented an online form for providers that need to report a complaint about a Medicare Advantage plan via cms.gov. Upon submission, these complaints will be sent to the Health Plan Management System (HPMS) Complaints Tracking Module (CTM).
