BY LeadingAge

On Thursday, July 21, 2022, a telehealth webinar will focus on the application of Telehealth/RPM in different care settings. “Telehealth in Practice: Driving Efficiencies & Improving Outcomes” takes place from 2 p.m. – 3:30 p.m. ET and will feature two provider use cases. Register today!

In the first use case, synchronous telemedicine/ telehealth is deployed in a skilled nursing community to improve access to physicians, reduce hospitalizations and re-admissions, and improve outcomes. In the second, remote patient monitoring (RPM) is being in home care to drive staff efficiencies, support care management, including chronic care management, and enhance engagement, even socialization.
