Provider organizations that missed the March 31 Provider Relief Fund (PRF) Reporting Period 4 (RP4) deadline due to extenuating circumstances have an opportunity to submit a late reporting request between April 17 and April 28 at 11:59 p.m. ET. This is a one-time opportunity. If Health Resources and Services Administration(HRSA) grants the request, providers are expected to submit their RP4 reports May 15 to June 2, 2023. Failure to submit required PRF reports is considered non-compliance with the PRF Terms and Conditions. Non-compliant providers will be required to return the PRF dollars they received and will be ineligible for any future funds. RP4 covers PRF and American Rescue Plan (ARP) Rural funds received between July 1 and December 31, 2021.
Eligible extenuating circumstances include:
- Severe illness or death – a key employee’s severe medical condition impacted availability to complete report (e.g., cancer treatment) or death of provider during RP4.
- Impacted by significant natural disaster – a significant natural disaster during or in close proximity of RP4 damaged the organization’s records.
- No reporting communications – an incorrect email address on file with HRSA prevented the provider organization from receiving email reminders prior to the RP4 deadline.
- Failure to submit – the provider organization registered and prepared its RP4 report in the PRF Reporting Portal but did not complete the final step to hit “submit” prior to deadline.
- Prior staff miscommunication – the provider organization believed it submitted its RP4 report based on an employee who is no longer with the organization who miscommunicated that the report was completed.
- Incomplete Targeted Distribution payments – a provider’s parent organization completed all general distribution payments but a targeted distribution(s) was not reported on by the subsidiary.

Steps for Requesting to Report Late Due to Extenuating Circumstances
Providers must follow the process below to submit a late reporting request. This is a one-time opportunity between April 17 and April 28 at 11:59 p.m. for Reporting Period 4. These dates will not be extended for submitting the request or submitting the late report. Providers should plan accordingly to ensure they have the necessary information and staff support to meet the deadlines.
- Step 1: If you have not done so already, register in the PRF Reporting Portal.
- Step 2: Submit your request via a DocuSign form. Providers may submit a request between April 17 and April 28 at 11:59 p.m. ET. A link to the DocuSign form will be available on Monday, April 17.
- Step 3: Your request will be reviewed and you will receive a notification informing you when you may submit your RP4 report via the PRF Reporting Portal. You will have approximately two weeks to complete the report. The projected dates for the submission of your report are May 15 to June 2, 2023.
