In early spring, Centers for Medicare & Medicaid Services (CMS) requested feedback on gaps in information sharing encountered by nursing homes during the hospital discharge process. We received a spirited response from our Nursing Home Network, which we shared with CMS.

CMS released QSO-23-16-hospitals on June 6, which reinforces hospital requirements when discharging to post-acute care. The memo highlights the issues with information sharing that we provided, and recommends resources to assist hospitals with ensuring accurate and thorough discharge information.
