A Supportive Services Plan describes in detail a HUD-assisted property’s capacity to connect residents with health care providers, educational programming, supportive services for Activities of Daily Living (ADLs), Instrumental Activities of Daily Living (IADLs), and other resources.
The key to a strong supportive services plan is connecting the needs of the affordable housing property residents to the services provided by the property or by the broader community.
