LeadingAge CAST has updated its Shared Care Planning and Coordination tool. The tool offers valuable information to help aging services providers learn about, choose, and implement these technologies designed to create personalized, comprehensive, and detailed care plans to help older adults age in place. Using these technologies, providers can help clients manage chronic diseases, offer post-acute support, and coordinate support services among multiple caregivers or care providers.
The benefits of care planning and coordination technologies include not only allowing older adults to remain in their homes as long as possible, but may also include reduced hospitalizations, emergency department visits, and laboratory tests.
The updated tool includes a white paper with background on the technologies. The tool’s interactive guide, online selection tool, and technology matrix help providers review technologies currently on the market and choose the ones that best meet their organization’s needs. The CAST tool now includes updates to the online tool and product matrix, incorporating new information on 10 products that were already part of the tool, and two new products that have been added:
